1. Company & Service Structure

  • Optometry & Vision Services
    Optometry and ocular wellness services are provided by Opto Bar Optometric, Inc., owned and operated by Dr. Lilan Le, OD.

  • Aesthetic & Medical Services
    Aesthetic and medical services, including IPL, radiofrequency, and injectables, are provided by LeBae under the guidance and supervision of Neal Kumar, MD, Board-Certified Dermatologist.

By booking or receiving any service, you acknowledge that care may be rendered by different licensed entities, depending on the nature of your treatment.

2. Acceptance of Terms

By accessing our website, booking appointments, or receiving services, you agree to abide by these Terms & Conditions.
We reserve the right to update these terms at any time. Continuing to use our website or services after changes are posted constitutes acceptance of the revised terms.

3. Appointments, Cancellations & Late Policy

  • A valid credit card is required to hold all appointments.

  • 24 hours’ notice is required for rescheduling or cancellation.

  • Cancellations with less than 24 hours’ notice or missed appointments will incur a $50 cancellation fee.

  • Arriving more than 10 minutes late may result in a shortened or rescheduled appointment.

  • SKeye reserves the right to modify or reschedule appointments due to provider availability, medical suitability, or safety considerations.

4. Payments, Packages & Refunds

  • Payment is due at the time of service unless prior arrangements are approved.

  • We accept credit/debit cards, HSA/FSA, and select financing options.

  • All services, products, packages, and gift cards are final sale and non-refundable.

  • Prepaid packages expire 12 months from the date of purchase.

  • At management discretion, remaining credit may be applied to alternate services of equal or lesser value.

  • Memberships are available for optometry services only. Aesthetic packages are sold as pre-paid sessions and cannot be converted to memberships.

5. Promotions, Gift Cards & Credits

  • Promotional discounts, referral credits, and gift cards have no cash value and cannot be combined with other offers.

  • Lost, stolen, or expired gift cards will not be replaced.

  • Promotional values expire according to the stated offer date; the underlying cash value (if applicable) never expires.

6. Medical Disclaimer

Our website and materials are for informational purposes only and do not constitute medical advice.
All patients must undergo a professional consultation prior to treatment to determine eligibility.
Results vary between individuals. No guarantee is expressed or implied.
Some treatments may not be suitable for pregnant or breastfeeding individuals, minors, or patients with certain medical conditions.

7. Minors & Eligibility

Patients under 18 years of age must have a parent or legal guardian present to consent to treatment.
Some procedures are restricted based on age, provider discretion, or medical history.

8. Treatment Expectations & Risks

  • Individual results depend on anatomy, skin type, lifestyle, and adherence to post-care instructions.

  • Side effects, including redness, swelling, or mild discomfort, may occur and are typically temporary.

  • Maintenance sessions may be recommended to sustain results.

  • Patients agree to follow pre- and post-treatment guidelines provided by their practitioner.

9. Practitioner Discretion

Providers may decline or modify treatment plans at their sole professional judgment to ensure patient safety, ethical practice, and the best possible results.

10. Photography, Video & Marketing Use

By receiving services, you grant permission for anonymized photographs and videos of treated areas to be used for educational or marketing purposes, unless you opt out in writing prior to your appointment.

11. Privacy & Data Protection

  • Patient and personal data are handled in compliance with applicable laws, including HIPAA where relevant.

  • We collect limited information for scheduling, billing, and communication purposes.

  • Your data will never be sold or shared without consent, except as required by law or for legitimate business operations.

12. Website & Third-Party Links

Our website may link to third-party sites or contain embedded content (such as booking widgets or payment portals).
SKeye is not responsible for the content, security, or privacy practices of these external sites. Use of third-party services is at your own discretion and risk.

13. Electronic Communications

By contacting us, booking online, or subscribing to our newsletter, you consent to receive communications by email, text, or phone regarding appointments, updates, and promotions.
You may unsubscribe or opt-out at any time.

14. Liability Waiver

To the fullest extent permitted by law, SKeye, Opto Bar Optometric Inc., LeBae, their owners, employees, and affiliates shall not be liable for indirect, incidental, or consequential damages related to services or the use of our website.
By undergoing treatment, you acknowledge and accept inherent risks and agree to release and hold harmless all affiliated providers.

15. Dispute Resolution & Governing Law

These Terms are governed by the laws of the State of California.
Any dispute arising under or in connection with these Terms shall first be attempted to be resolved informally. If unresolved, it will be submitted to binding arbitration in Los Angeles County, CA, in accordance with the rules of the American Arbitration Association.

16. Severability

If any part of these Terms is found invalid or unenforceable, the remaining provisions will continue in full force.
